BEFORE BOOKING A TATTOO APPOINTMENT

**You must be 18 years of age or have a parent present for consent (with ID) before any tattoo procedure, including Microblading and SMP.

All cosmetic and body tattoo procedures cannot be done on anyone who is:

• Pregnant

• Sunburned or tanned with 1 week

• Currently undergoing chemo or radiation

• Has seizures

• Has had surgery within 1 month of appointment

• Has not been off Retin-a in treatment area for at least 4 weeks

• Has not been off Accutane or Chantix for at least 1 year

• Has hemophilia

• Has a pacemaker

• Prone to keloid scarring

• Actively taking blood thinners or certain medications
